Your CAM is not correctly decrypting the above channel for some reason.
Have you copied the complete log? It should be a bit more verbose 
like this:

debug: opening MMI session (5)
debug: sending DisplayReply on session (5)
debug: MMI title:
debug: MMI subtitle:
debug: MMI bottom:
debug: MMI choice: You are not entitled to receive this programme
