[dvblast-devel] /emm and /ecm no longer exist

I do not plan on reintroducing these flags. In the long run the plan is to add /nocam and /cam options, which disable/enable the use of the CAM for descrambling the stream. If the stream is sent out scrambled, ECMs and EMMs should be automatically added (otherwise we are not compliant BTW).

However, this is too complex to implement with the current way of selecting PIDs and CAPMTs, so I have to rewrite and simplify large portions of demux.c (and perhaps split the file, which is getting too big). That's why I'd like to release 2.1 before breaking everything.
