Look at the DATN experience -they use 1 X Serve box per high  
bandwidth stream, looking to go to 2 streams per box
soon. Dig into

http://datn.wisc.edu/about/

This uses QTB.

They also have implemented scripts to reboot streams that die, etc.,  
for high availability.
