Now a little explanation (forgotten in the commit message):
Added ts-dump-file and ts-dump-append to ts demuxer implementing similar 
functionality of the demuxdump demuxer. Actually that is more or less a 
copy and paste. Except for one added feature that allows you to decrypt 
an encrypted CSA stream while dumping. For that to work the user needs 
to use the --ts-dump-file in combination with --ts-csa-ck=<key> switch 
with the ts-demuxer. The resulting "dumpfile" will contain the decrypted 
MPEG2-TS stream.

A warning about --ts-dump-size do not use, because there are some issues 
with it.
