 For vout: vout_Request will recycle the provided vout if possible or destroy

 For aout: it is always reusable and can be used by multiple decoders at the same
time. There should be no reason to ever need to recreate one.
 Instantiation of 2 outputs is possible but is really not 'standard'.
(I only see one need, to output 2 audio at the same time on 2 distinct device,
but nothing allows to do it except maybe vlm).
