You can either use CoreText/Uniscribe or HarfBuzz' native shaping, and
only native shaping requires ICU (or glib, or whatever). However, the
CoreText/Uniscribe backends are supposedly not production ready, for
testing only and might be removed at some point. At least that is what
Behdad says, the HarfBuzz developer.
