Plays fine with --avformat demux (before and after patch), however
normal playback fails with:

[utvideo @ 0x7f1670ca7840] Insufficient extradata size 0, should be at least 16
[00007f1670c83278] avcodec decoder error: cannot open codec (Ut Video)
[00007f1670c83278] core decoder error: Codec `ULRA' (Ut Video) is not supported.

whereas before the patch it failed earlier with:
[00007f1d38c832b8] core decoder error: Codec `ULH2' (No description
for this codec) is not supported.

That said, if it's just a bug in the AVI demuxer maybe this should be
merged anyway?
