I am also having problems with the h264 files encoded by mencoder.  When
I try to convert the h264 files created with mencoder (whether rawvideo
or avi) to mp4 the bitrate, max bitrate and the decoder buffer size are
all zero (according to MP4Box).  To remux the file, you need to set
-ffourcc 'avc1' when not using rawvideo or ffmpeg creates a stream with
the wrong fourcc.  These files crash QuickTime Player
