It would be interesting to see if PiP video and main video are both
found from the same clip and mplayer just selects wrong video track. I
belive mplayer changes video track with "_" key.

What PID it selects for video ? It should select 4113 (0x1011), like:
  "VIDEO VC1(pid=4113) AUDIO A52(pid=4353) NO SUBS (yet)!  PROGRAM N. 1"

Can you get dump/trace of PMT contents ? (mplayer -msglevel all=6 ?)
