Let me run it with highest verbosity. As I cannot consistently reproduce the problem, I would need to wait for a day or two until the problem occurs.

Could you post the log out? With highest verbosity set. Also providing a
sample file could be helpful.
Wrong PTS, a slow computer or the classic 'late picture skipped' bug in
case of low frame rate all could lead to this.
