This was exactly what was going on. And it is (as far as I understand) a
common problem when dealing with set top boxes and similar. The forum poster
concluded that mpeg2ts performed far better when his clock was faulty. (and
yes, I did say faulty, not "not adjusted recently" in my previous post).
