I should add that I've actually seen one case where I had to change
level_idc (e.g. x264 --level 41 or lower) to get something to play a
stream generated by x264.

I have no idea how common it is; hopefully most players would ignore the
signalled level and just try their best to decode, but there's at least
a slim chance this is your problem.
