OK, I've munged your patch into the attached, which makes it only one
option with three modes (off, strict, normal).

However, it appears that the patch doesn't work correctly with
"strict" B-pyramid and fewer than 4 refs; it produces invalid output
(JM agrees).  Do you know what the problem is?
