[x265] LSMASH integration

LSMASH wouldn't go into the x265 code itself, the x265 output stream would
be adjusted so that it can do more than file output for the resulting
stream. Once that functionality is added the head and can then be pulled
into a 'lsmash' branch where lsmash can be shimmed to it as the output
muxer. 

The x264 equivalent is the unofficial 'lsmash' branch that pulls from the
'origin/head' of the official x264 repository.

The result is that you can make an output that muxes the h.265 ES and an
audio ES from the x265 cli.
